FAX-TO-QUIT LOUISIANA helps you to provide an easy and seamless counseling component to patients who are ready to conquer their tobacco addictions. Simply fax the Quit Louisiana referral form to the Louisiana Tobacco Quitline. It’s just that easy. A cessation counselor will then call the referred patient who is eligible to receive 5 free telephone cessation-counseling sessions; 8 if she is pregnant.  Fax-to-Quit referral forms and a training manual may be obtained by calling Tiffany Netters at 225.342.2664.

        ASK - Identify tobacco users at every visit
        ADVISE - Talk with the patients about tobacco use and urge them to quit
        ASSESS - Determine if the patient is willing to make a quit attempt at this time
        ASSIST - Prescribe a medication unless contradictions exist
